CURBSIDE COMFORT WAIVERS & RELEASES
Surf Dog Ricochet photo/video release
I understand, acknowledge and agree to the following:
I understand that photographs and videos taken of me in relation to Surf Dog Ricochet and Cori, may be used for fundraising, educational, and promotional purposes, and/or to create awareness of initiatives and fundraising activities through all media including television, internet, video, radio, social media, press releases, YouTube, websites and any media now known or hereafter devised, and may be submitted to related publications and various other media works. I waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith.
I additionally authorize and give my full consent to Surf Dog Ricochet/Cori to copyright and/or publish any and all photographs, videotapes and/or film in which I appear without limitations or reservations. I also permit free use of my name, video, pictures of me and/or my child(ren) for Ricochet/Cori's initiatives and fundraising/promotional materials.
I release Judy Fridono and any photographer/videographer that captures my image to use the pictures/video in association with promotion of their work, and/or to post on websites.
This agreement shall be interpreted by the laws of the State of California and the parties herein subject to jurisdiction with the State Courts of the State of California with venue in San Diego County, California.

Surf Dog Ricochet release of liability for participant
In consideration for being allowed to participate in any way with Surf Dog Ricochet and Cori, I and/or the minor participant for whom I am the natural parent and/or guardian ad litem, on behalf of myself and/or the minor participant's heirs, assigns, personal representatives and next of kin, hereby acknowledge and provide my legal consent to the following:
I/we have read the above waiver and release, understand that I/we have given up substantial rights by signing it, have not changed it orally, and sign it voluntarily.
- Acknowledge and fully understand that I and/or the minor participant, will be engaging in dog related activities which include, but are not limited to petting, walking, performing tricks, giving treats and other activities, and that these activities are hazardous and carry with them a risk of injury not only from my own actions, but from actions and/or interactions with the dogs, their handler, Judy Fridono and/or others, or the condition of the premises or any equipment used. Importantly, because participating in dog related activities such as interaction with the dog carries associated risks due to an ever changing environment, I hereby waive any claims of liability against Judy Fridono for risks or events unknown or which are otherwise unforeseeable to me at this time.
- Indemnify and hold harmless, Judy Fridono against any damages or liability incurred while participating in the session or arising from any claims, demand, action, or cause of action by the participant resulting from any injury that may occur to the participant while participating in the session.
- Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
- The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Judy Fridono has put in place the following preventative measures to reduce the spread of COVID-19; however, she cannot guarantee that you or your child(ren) will not become infected with COVID-19.
- I voluntarily agree to abide by the following criteria during the curbside comfort session:
• Visits must be held outdoors
• Individuals must not be sick
• Individuals must not have been exposed to anyone with covid-19 in the last 14 days
• Individuals must practice social distancing and remain at least six feet away from each other
• Individuals must wear masks
• Hands must be washed and/or sanitized before the individual approaches Ricochet
• Disposable gloves must be worn before touching Ricochet
• No face-to-face contact
• Gloves must be removed and disposed of immediately after the interaction
• Hands must be washed and/or sanitized immediately after removing the gloves
• Slip lead must be used during interaction and washed afterwards - I voluntarily agree to all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, covid-19, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s participation in curbside comfort sessions (“Claims”). On my behalf, and on behalf of my children, I hereby release, waive, indemnify and covenant not to sue, discharge, and hold harmless Judy Fridono from demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
I/we have read the above waiver and release, understand that I/we have given up substantial rights by signing it, have not changed it orally, and sign it voluntarily.
